Sustainability and Transformation Plan

In common with health and care organisations throughout England, we have been working with our partners across South Gloucestershire, Bristol and North Somerset to develop a sustainability and transformation plan.

The plan will sets out how health and care services will work in partnership to develop plans that will enable us to continue to meet the health needs of our local population at a time of increasing demand and constrained resources.

This sustainability transformation plan summary describes how we propose to organise and deliver services in a different way to meet the needs of the population across South Gloucestershire, Bristol, and North Somerset.

How can I get involved?

We are inviting you to get involved in the ongoing development of the South Gloucestershire, Bristol and North Somerset Sustainability and Transformation Plan. The publication of the draft STP is your chance to get involved at this early stage. Your feedback is important. We don’t yet have all the answers but this is the start of a conversation to develop the plan with your input.
