How to make a referral

Our placements

We offer placements that meet the needs of the service users on an individual basis. This can be both in terms of activities and outcome focus, but also the length of weekly session and how long each placement lasts. It is possible to change the amount of contact hours between a mentor and service user to reflect changing circumstances or interests. We always work to meet the needs of all interested parties. We do this by talking over the reasons for the referral, asking all parties for their preferred outcome and coming to a common agreement.

Who do we work with?

We work with people of all ages from preschool children to those in later life. We are all different and we recognise this. We take the time to get to know each of our service users and their needs. These can include Autism, physical and learning difficulties, poor mental health, poor physical health, loneliness and isolation, educational issues, employment support, anti-social behaviour, dementia and those going through a difficult time who just need a bit of extra emotional support. We welcome anyone into our service and are here to provide support.

Referral form

In order to make a referral, please download and complete the Breakthrough referral form (in the downloads section on the right-hand side of this page).

Depending on the type of organisation the referral form is received from, we will require you to send the form via the following:

  • South Gloucestershire Schools: Please send via the SOFIE secure system (secure communications system)
  • South Gloucestershire Academies:Please call 01454 864449 and we will advise on the appropriate method for sending the form.
  • Internal South Gloucestershire Council departments: Please send to Breakthrough@southglos.gov.uk
  • External organisations/individuals: Please call 01454 864449 and we will advise on the appropriate method for sending the form.

What happens when a referral is received?

When we receive a referral, our coordinators will review the form and make contact with the initial referrer to discuss how we are able to support. We will then meet with the service user and look at matching them to a suitable mentor, looking at interests and potential activities for the placement. We will then introduce the service user to the mentor in advance of the placement beginning. For more information on this process, please see our referral process diagram.

Funding

Each Breakthrough placement needs to be funded in order for us to provide support to a service user. We can support individuals as part of their Direct Payments allowance, as part of contracts within organisations, schools and academies or we can arrange individual payments if you are self-funding.
