These notes are for any worker from any agency working with young people, who needs guidance in filling in a Request for Single Service form where the issue is either a young person aged 16-25 who is homeless or at risk of homelessness and is in need of a Homelessness or Prevention Assessment by South Gloucestershire Council, or a Joint Homelessness/Social Services Assessment if the young person is aged 16/17 or previously in care.
