People with a developing serious mental illness should attend their GP who may refer them to the Primary Care Liaison Service. The AWP South Gloucestershire Mental Health Locality Unit consists of different teams which provide services to older and younger adults. The Older Adult teams work in partnership with South Gloucestershire Council and in the Adult teams AWP provides an integrated service including social workers, community psychiatric nurses, occupational therapists, psychologists, psychotherapists and psychiatrists.
All teams use the integrated Care Programme Approach. Practitioners work closely with the Service User, their family and carers who together formulate a care plan that meets the needs of the Service User which supports them and their family / carer in their recovery journey.
Each Service User will have a named Care Coordinator who will provide regular contact and will incorporate into the care plan how to get help in an emergency.
The care plan will include help with your mental health needs but can also include support with practical help, adult care services, advice on housing, employment, personal relationships, benefits and other money matters.
