So that you can get a service such as home care, you will need to have an assessment. Assessments are about finding out what needs you have and what services would best meet those needs. Someone will talk with you and perhaps your carer, to identify and understand your needs so that we can provide services best suited to you.

Aged 65 or over
If you are aged 65 and over, staff in the health service, Community Care and Housing Department will work together to assess your needs. This is known as a 'single assessment process'.
