If you are working with a child or young person who has a clearly identified need which can be met by one service, you need to complete the 'request for single service' and send it to the receiving service. You no longer need to send copies to our Bowling Hill office. To complete a 'request for single service' form, follow the link under the 'Do it now' section of this page.
All services and agencies involved with the child or young person must retain responsibility for reviewing and monitoring progress. If the child's needs remain unmet, a 'common assessment framework' (CAF) must be considered.
