In its policy document ‘A Vision for Social Care: Capable Communities and Active Citizens’ the Government says that people with learning difficulties need person centred planning in order to have more choice and control.
Person centred planning is a way of helping people to think about what they want now and in the future. It is about supporting people to plan their lives, work towards their goals and get the right support.
It is an approach that means planning is done with a person - not for them. There are lots of ways that person centred planning can happen but if you need to plan some support the main things to remember are that,
• You are the most important person.
• It is not a one off meeting, but it should carry on through your life.
• It is a way of working together to make changes.
• You can make sure that the people who are important to you become partners in the planning process.
