Flutissimo enrolment application form

To enrol on the Flutissimo Ensemble, please download the form below and save to your computer. The latest fees are indicated on the form. No charge will be made if you are in receipt of Income Support/free school meals (you will need to confirm this with the director of the ensemble).

Completed forms should be handed in at the ensemble group meeting or sent to the music service.
