Supporting People - financial forms
It is important that you notify us of the start and cease of Supporting People services using the forms listed in the ‘Read more’ section of this page.
All information marked with an asterisk must be completed before sending to the Supporting People Team.
Notifying us of new service users
The Supporting People Team should be informed of all new service users within 4 weeks of the start of their service by completing the relevant SP1 form:
Short Term Services (Support lasting up to 2 years):
Please notify us of the new service user by returning the SP1 – Short Term Services Start Form. Short Term Services are not chargeable.
Long Term Services (Support for more than 2 years):
Please complete the SP1 – Long Term Services Start Form. Service users should be advised that they will be receiving a chargeable service and that they will be invoiced for the service unless they are receiving housing benefit or have been assessed as a 'NIL charge' via a 'fairer charging assessment'.
Notifying us of a service being ceased
You will need to notify the Supporting People Team of a service being ceased by submitting the relevant SP2 form for either Long or Short term services; this includes notifying us of the death of a service user.
Monitoring your payments
Service Providers are sent a schedule of payments with each four weekly payment from Supporting People. These schedules detail which service users are being paid Supporting People subsidy. Providers should check these schedules carefully and notify us of any amendments or updates.
