It is important that you notify us of changes to any of the below.

Block subsidy/block gross chargeable contracts - new users and requirements

The Supporting People Team should be informed of all new service users within 4 weeks of the start of their tenancy by completing the SP1 form. Service users should be advised that they will be receiving a chargeable service and will be invoiced for the service unless they are receiving housing benefit or have been assessed as a 'NIL charge' via a 'fairer charging assessment'. We will need a data consent form for each service user applying for housing benefit. All information marked with an asterisk must be completed before sending to the Supporting People Team.

Service user/tenancy changes

You will need to notify the Supporting People Team of any changes or tenant movements within schemes within block subsidy or block gross chargeable contracts. This should be done by completing the SP2 form. If they are relocating to another scheme within South Gloucestershire, then we will require a new SP1 form for their new tenancy.

Tenancy ceased

Please complete an SP2 form for all tenancies that end. If the tenant has died we will require an SP3 form.

Monitoring your payments

With each four-week payment providers with block aubsidy contracts or block gross chargeable contracts are sent schedules. These schedules will give details of service users for which a Supporting People subsidy is currently being paid. Please check these schedules carefully and notify us of any amendments or updates.
