Children's Services - requesting single service

If you are working with a child or young person who has a clearly identified need which can be met by one service, you need to complete the 'Request for single service' form and send to the receiving service. You no longer need to send copies to Bowling Hill. This replaces the previous MARS form.

All services and agencies involved with the child or young person must retain responsibility for reviewing and monitoring progress. If the child's needs remain unmet, a Common Assessment Framework (CAF) must be considered. Notes

  • Portage Service – referrals can be made by parents and professionals working with the child via a service referral form
  • Preschool liaison teachers and area SENCOs (special educational needs co-ordinators) – referrals can be made by staff in settings and professionals working with the child via the service referral form
  • Referrals must be agreed with parents. Children with statements of special educational needs may be supported by the early years inclusion manager
 

What you can expect

Completed single service request forms should go to the receiving agency (check with them whether they want to receive hard copies or can receive email copies).
