Hospital Liaison Nurses help people with learning difficulties who have to go to hospital for an outpatient appointment or longer stay.
Hospital Liaison Nurses can:
- visit people at home to talk to them about their appointment and see how to make things better at the hospital
- show people around the hospital and where to go for their appointment
- take people to meet staff who will be looking after them
- tell hospital staff about any needs people have, before they go to hospital
- support people at their outpatient appointment or during a longer stay in hospital
- make sure that when people leave hospital those who help to look after them, such as family and community teams, have the right information about what has happened
