Complete request for single service form

If you are working with a child or young person who has a clearly identified need which can be met by one service, you need to complete the request for single service and send this to the receiving service. You no longer need to send copies to Bowling Hill.

All services and agencies involved with the child or young person must retain responsibility for reviewing and monitoring progress. If the child's needs remain unmet, a common assessment framework (CAF) must be considered.

 

What you can expect

This form is for use when a child or young person has one clearly identified need which can be met by one service. Please note all referrals to acute health services (such as community paediatricians, CAMHS, speech and language therapy etc.) must be directed through the GP, health visitor or school health nurse.

Do not use this form but complete a CAF if:

  • You believe a child or young person will not progress towards the five 'every child matters' priority outcomes without additional services
  • The child or young person's needs are unclear or broader than your service can address and a CAF would help to identify the needs
