How to apply for floating support

Application forms are available:

  • From the council's one-stop shops in Yate, Kingswood or Thornbury
  • By telephoning the Access Service on 01454 865543
  • By downloading and completing the printable application form in the 'Read more' section on this page
  • Alternatively, complete the on-line form below

The Floating Support access service acts as a single point of access for all applications. Applications should be completed by or with the person requiring support.

Please give as much information as possible to enable your request for support to be sent to the appropriate agency.

 

What you can expect

You will receive a response from the access service within 3-5 working days of receipt of the application.
