Referral process

This is a summary of what happens after the intial referral;

•The referring organisation makes initial contact with mentee

• The referrer gives details to Breakthrough Coordinator
• Co-ordinator meets with referrer, then gets introduced to the mentee and parents/Guardians if possible
• Breakthrough is explained to all and child and parent information flyers are given out
• An appropriate mentor is sourced
• Mentor is introduced to referrer, mentee and family. All contact numbers are exchanged
• Likes and dislikes are discussed and all contact numbers are exchanged

• Goals for the sessions are set
• Complete registration
• Department of Health Strength & Difficulty questionnaires completed
• First session is arranged

What is explained to the mentee about breakthrough?

• Why they are on the Project
• The relationship between the mentor and mentee and boundaries
• Confidentiality and Safe Guarding. It is explained that anything said is confidential, however it will passed on to Breakthrough if it is anything that puts them at risk
• How long the sessions will be and for how long they will get mentoring
• What the session will involve
• Rewards and expenditure
• Any questions 
 

Co-ordinators

• The Co-ordinators contacts are given to all involved and can be contacted about any issue at any time
• Co-ordinators will visit the placements see to how they are going
