Before a hackney carriage or private hire licence is issued you will need to have a medical examination. This is carried out by a council-appointed doctor at the Courtside Surgery, Kennedy Way, Yate. Application
We will issue you with a medical form. You need to return the completed form with a passport-sized photograph directly to us. We will then forward these documents to the Courtside Surgery and the surgery will contact you to arrange a medical examination. They should do this within five days.
You must pay for the medical examination yourself. The fee for a standard medical examination is £56.95 and, if an ECG (electrocardiograph) is required, a further £10 will be payable. You must pay before the examination takes place. After the medical examination the doctor will complete and send a report to us. The report will be examined as part of the application process.
